Applications can be picked up at the Town Clerk’s Office. They should be taken to your doctor for his diagnosis and signature. Please make sure your doctor includes the amount of time that the permit is to be used. The longest time period for a permit is three years. We also issue temporary permits. Once the forms are returned to the Town Clerk’s office, we will issue a hangtag which can go in any car that you ride in. These permits cannot be used by anyone other than the permit holder.
